Health Information Management - QCI clerk Full-time - Evening shift

Salary Range: $29.70 - $36.87 plus applicable shift differential
General Description
The Quality Control Indexing (QCI) Clerk is responsible for analyzing medical record documents and documentation to ensure complete medical record information and accurate medical record patient identification and document indexing. Chart correction staff are responsible for performing merges of duplicate medical record numbers, addressing requests for correction of errors, opening chart correction cases when appropriate and facilitating correction of identified errors in the medical record. Process release of information (ROI) requests as needed.
Essential Job Functions
- Conducts quality review to ensure that all patient records and loose documents scanned into OnBase are of the highest possible level of quality.
- Indexes all documents to the appropriate patient encounter and appropriate document type.
- Reviews each patient’s record for incomplete or missing items, identifies the responsible practitioner for each of these items, and assigns and links the deficiencies in Epic to the appropriate physician for completion.
- Notifies physicians of pending records that need completion in a timely manner, identifying those physicians who will be suspended and notifying the appropriate hospital personnel/departments of physician suspensions.
- Performs merges of duplicate medical record numbers.
- Facilitates correction of identified errors in the medical record.
- Collects, enters and reports accurate and complete statistical data as needed or assigned.
- Process release of information (ROI) requests as needed.
- Assures patient safety, privacy and confidentiality as appropriate or required.
May also perform other duties as assigned.
Qualifications
1. Education -
Required: High school graduate.
Preferred: Satisfactory completion of a course in medical terminology or health information management.
2. Licenses/Certifications –
Required: None
Preferred: RHIT or RHIA
3. Work Experience –
Required: Previous medical records experience.
Preferred: Previous experience in a health information management department.
4. Skills/computer/specific technical –
Required:
A. Excellent organizational skills and ability to work well independently.
B. Familiarity with databases and data entry.
5. Other qualifications –
A. Ability to work well and communicate effectively with persons of varying educational and cultural backgrounds
