What You Will Do:
Assesses, plans, implements and evaluates the delivery of resident care. Ensures that quality care is rendered to all resident/patient in accordance with the Nurse Practice Act, and licensing agencies' regulations utilizing established standards of care for the nursing department. Participates in quality review, credentialing and education activities. Promotes collaborative practice with the Medical Staff. Communicates pertinent information to Director of Lynn Care Center Dana, make sure you are consistent with chain of command and CNA's. Accurately completes MDS's, RAPS and Care Plans for unit. Functions as charge nurse for unit. Performs other related duties as assigned.
Two years of Long Term Care experience preferred
Certification & Licensures
LPN Nursing License in the State of Virginia required
BLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved required *

Knowledgeable in the cognitive, physical, emotional and chronological stages of human growth and development. Responsible to perform psychological and social, and age appropriate assessment, care, and treatment of resident/patient. Positions in the following areas require a criminal background and/or central registry check(s), and continued employment is based on the outcome of the report(s): Women's Care Center and Lynn Care Center. Employee must obtain annual flu vaccination. Position requires a nurse that is flexible, able to work in all units of Lynn Care Center (Skilled, Dementia, Nursing Facility and Private Units). Willing to report on their scheduled shift to whichever unit assigned as needed for that particular shift/day.
