What You Will Do:
Reporting directly to the VHS Chief Population and Community Health Officer, the Director, Integrated Care Coordination, is a facilitator leader responsible for leading efforts to design and develop care management services for Valley Health. The Director informs and executes the strategic direction of care management for Valley Health and is responsible for the development, implementation, and monitoring of the day-to-day operations and employees for integrated care coordination delivery. This role provides leadership to all inpatient care coordination across Valley Health hospitals, which includes RN Care Coordination and Discharge Planning, Department of Social Work, Non-licensed Discharge Planning, Outpatient Care Coordination to reduce hospital readmissions, and is a primary liaison to Utilization Management through Valley Health's clinical partnership.
As a senior member of the Community and Population Health Leadership team, the Director provides a dotted line leadership and oversight to outpatient care coordination provided to Valley Health attributed lives in the Employee Health plan, ACO and future risk-based products. The Director has primary responsibility for inpatient length of stay (LOS) and readmissions as well as the responsibility to form collaborative relationships with peers in nursing and post-acute services to ensure care coordination and effective resource management across the continuum of care.
What You Will Bring:
Education
Bachelors Nursing, Healthcare Administration, Business or related field required
Masters Nursing, Healthcare Administration, Business or related field preferred
Experience
Prior care management experience required
Qualifications
Demonstrated skills in planning, organizing and managing professions/specialties in complex functions and processes required
Understanding of hospital finance, third party payers and reimbursement models required
Ability to articulate ideas and concepts concisely and effectively in writing and verbally required
Demonstrated ability to work in a matrix environment demonstrating collaboration and the ability to prioritize for outcomes required
