What You Will Do:

Clinical Team Assistant collaborates extensively with the home health clinical team of registered nurses, physical therapists, and occupational therapists by processing patient information including medication and order entry into the electronic medical record (EMR) upon referral for home health patients to improve quality and completeness of documentation of care provided; supports timely, accurate, and complete documentation of clinical information used for measuring and reporting facility outcomes. In addition, team assistants perform intake functions by entering into the EMR patient demographics, insurance payers, and relevant medical and psychosocial information as needed. Other responsibilities include: performing chart audits and other related PI activities; participating in daily huddles and managing phone calls from patients, staff, referral sources, and patient families.

What You Will Bring:

Education

High School Diploma or GED required
Accredited medical secretarial or business program graduate preferred.

Experience
Two years experience in medical/hospital setting preferred
Home health experience preferred


Qualifications
Excellent organizational skills with attention to detail required
Ability to establish and maintain effective working relationships with internal and external customers required
Excellent oral and written communication skills required
Must not have been convicted of or have pending charges of any barrier crime.
