What You Will Do:

The Mobile Integrated Healthcare- Community Paramedic (MIH-CP) will work as an integral part of an interdisciplinary care team within Valley Health and additional healthcare stakeholders to improve patient outcomes and achieve strategic goals. The MIH-CP will be a highly motivated and professional individual capable of collaborating with a complex group of healthcare and community resource providers to set and achieve goals for patients enrolled in the MIH-CP program.
General Duties and Responsibilities
Examines, screens, treats and coordinates health services for patients as permitted within state scope of practice, protocols and procedures. Intervenes as needed for chronic and acute medical needs per protocol and scope of practice. Assists the provider in evaluating patient responses to clinical condition and effectiveness of treatment as well as in assessment of the physical, psychological and social assessment of patient. Effectively communicates, obtains and interprets information in a manner appropriate to the age and development level of the patient. Ensures timely entry of all charges into the patient record. Reviews discharge information with patient and assess for understanding as needed. Adheres to National Patient Safety Goals as they apply to the Ambulatory Care setting. Provides competent assessment, care and treatment appropriate to the age of patients served as detailed and determined through the unit specific Scope of Care, Competency/Skills Checklists, and In-service Training. Conducts post-hospital release follow-up care, including but not limited to, monitoring medication, health literacy and knowledge of current condition(s), dressing changes, checking vital signs and carrying out physician orders as needed. Observes, records, and reports to physicians, patient’s conditions and reactions to drugs, treatments, and significant incidents as well as identifying and addressing barriers related to social determinants of health. Conducts holistic evaluation of clinical, physical, social, environmental and psychological factors to develop patient centric goals to improve self- management or establish long term case management Conducts patient education on chronic diseases including diabetes prevention/treatment, hypertension, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), Pneumonia, falls assessment, injury evaluation, geriatric frailty visits, nutrition and more. Administers patient care consistent with department protocols and physician orders. Coordinates appointments and follow-up with physicians, clinics, social services, community organizations and hospitals. Develops and completes appropriate reports and templates for the MIH-CP Program. Attends meetings and delivers presentations as requested and available. Safely operates and maintains MIH-CP vehicles. Adheres to all safety policies, procedures and protocols. Maintains required medical supplies and equipment.

What You Will Bring:

Education

High school diploma or GED required.
Associates degree preferred

Experience
Minimum 5 years of EMS experience with at least 3 years as a Paramedic required
Training, CQI, marketing and/or public information experience helpful

Certification & Licensures
National Registry Paramedic Certification required
Virginia State Paramedic Certification required required within 30 days of hire
Community Paramedic Certification (CP-C) from International Board of Specialty Certifications (IBSC) required within 60 days of hire
BLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved required*
ACLS Certification (Advanced Cardiac Life Support) required
PALS Certification (Pediatric Advanced Life Support) required
Valid motor vehicle operations license required

* New hires must have American Heart Association (AHA) appropriate certification prior to completion of orientation.
EVOC (Emergency Vehicle Operators Certificate) or approved equivalent required.
DOT preferred

Qualifications
Professionalism and ability to be discreet with confidential and sensitive issues required.
Strong verbal communication, written communication, and organizational skills required.
Ability to handle multiple tasks, projects, and meet deadlines required.
