Job Summary
Reporting to the Chief Operating Officer of VPE, the Medical Director - Primary Care will be the physician primarily responsible for driving quality improvement throughout the primary care medical group. As Valley Health continues to pursue opportunities in both pay for performance in value based care initiatives, the organization requires a physician liter to focus on improvements in these areas. The Medical Director will be an experienced physician leader with a demonstrated record of improvement in quality of care.
Specific responsibilities will include:
· Leadership of the primary care council which will include representatives from each of our communities who we’ll services ambassador’s to the providers in their communities
· Review of monthly data from payers and internal sources describing opportunities in value based care and pay for performance initiatives
· Identification of Analysis in these data sets
· Design of communication and action strategies to encourage other providers to improve the quality of care in these areas
· Review of team based interventions designed to improve outcomes
· Leadership of lean process redesign to improve processes in the practices that drive improved patient outcomes
· Leadership in the development of nurse navigation and chronic care management programs to improve the care of chronically ill patient’s between office visits and improve outcomes
· Participation in the Peer and Quality committee as a vice chair of the committee
· Represent VHS/VPE on the SHARE Quality Committee

Credentialed by the hospital to practice internal medicine or family medicine with appropriate Medical Degree/Diploma Program of Study Additional Information required

Five years or greater of primary care practice experience required
Physician leadership experience preferred

Certification & Licensures
Board certified in Family Medicine, Pediatrics, or Internal Medicine required

• Must have the ability to work well with interdisciplinary team of healthcare providers
• Must possess and maintain an unrestricted license to practice in the Commonwealth of Virginia or be eligible to obtain
• Must have professional liability insurance coverage in such amounts and at such rates acceptable to VPE
• Must have an unrestricted valid DEA number, to dispense controlled substances
• Must have participating provider status with Medicare, Medicaid, and other health plans or payors with which VPE has contracts
• Competent to provide assessment, care and treatment appropriate to the age group of patients served
