What You Will Do:

Utilizes skills and knowledge to assist the healthcare team in the clinical functions related to the medical office Provides basic therapeutic nursing care according to established standards of practice under the direction and supervision of a physician Must be capable to assist the physician and nurse practitioners in the physical, psychological, and social age-appropriate assessment, care, and treatment of assigned patients Provides care to those patients whose care requirements are routine and standardized with predictable outcomes Assists the physician and healthcare team to achieve desired patient outcomes Evaluates and maintains the environment to promote patient and healthcare team safety Demonstrates knowledge of and participates in performance improvement activities. Health Care Coach and Coordinator for Transitions of Care from acute to post-acute settings. Maintains competencies within practical nursing standards and assumes responsibility for self-development in life-long learning.
Two years of nursing practice in a hospital or clinic setting or experience with chronic disease management required Three or more years’ experience in a hospital or clinic setting and demonstrated strong leadership skills and experiences with coordinating care of chronic disease patients preferred

Certification & Licensures
LPN licensure required *
BLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved required **
* Based on primary state of residency and in accordance with current Virginia Board of Nursing Regulations, must be licensed or eligible to practice pending licensure as a Licensed Practical Nurse in the Commonwealth of Virginia and/or West Virginia (as applicable depending upon geographical area assigned) with either a: Multi-State License, under the Nurse Licensure Compact required ** New hires must have American Heart Association (AHA) appropriate certification prior to completion of orientation.

? Knowledgeable in the cognitive, physical, emotional, and chronological stages of human growth and development required ? Communicates effectively and works cooperatively with others required ? Ability to provide highest level of care to all patients of all age ranges regardless of illnesses, disease or physical limitations required ? Knowledge of funding, resources, clinical standards and outcomes for population required
