Care Management NP/PA, Complex Clinical Care Program

University Medical Partners (UMP), a partner of Stanford Medicine, is seeking a full-time BC/BE Care Management NP/PA to join our East Bay clinic network.
Position Details:
- Full-time opportunity – 40 hours per week
- The Care Management NP/PA is the primary point of contact for high-risk, high-acuity patients included in the Complex Clinical Care (CCC) program
- This hybrid (part remote part onsite visit) position involves providing care independently in various settings including but not limited to home visits, tele-visits, skilled nursing facilities and hospitals
- Maintains constant care throughout the patient’s experience which includes onboarding, episodic service, and standard follow-up
- Participates in establishing, modifying, monitoring, and aligning to standard policies, protocols, and practice management guidelines for Complex Clinical Care program patient population
- Responsible for initial assessment and Care Plan
- Reconciles and modifies medications and care in collaboration with the primary care physician and other providers
- Facilitates communication among patient, family caregivers, and health care professionals
- Coordinates and leads activities relating to outreach, healthcare provider promotion of the Complex Clinical Care Program and establishes effective networks
- Expected to learn, use, and apply principles relating to population management, quality principles and practices, motivational interviewing and care pathways in their daily work
- Seeking dynamic Nurse Practitioner with at least 1 years’ clinical experience, preferably in urgent, acute care or emergency care
- Must be able to function independently with minimal physician oversight
- Must have a passion for working with a population of complex senior patients
- Must possess high empathy and a commitment to deliver excellent quality and service
- Must have proven skills in effective outreach
- Must have respectful, direct, and clear communication
- May require travel throughout the South and East Bay Area
- Must be able to use standard health care process improvement methodologies (CQI, Lean, PDSA)
- In collaboration with the Medical Director, assures coordination and integration of quality, performance and process improvement plans and activities
- Knowledge of Medicare and Managed care rules and regulations
- Flexibility with job assignments is required including duties as assigned
- Epic EMR experience a plus
