Summary: The Nurse Case Manager maintains the responsibilities of Utilization Review and Discharge Planning for all hospital patients under CMS Conditions of Participation Regulations. This role is responsible for screening all patients against level of care criteria to qualify the level of care for hospitalization, monitor continued stay and patient response to interventions and the plan of care, develop appropriate discharge plans for all patients who meet high risk screening criteria as well as those who do not if one is requested by the Attending Physician. When the patient is ready for discharge or transfer to the next level of care, it is the Nurse Case Manager who works to develop those plans within the healthcare team and maintains responsibility for the safe communication and implementation of the plan at the time of discharge. The Case Manager must assess every patient against all elements of Federal and State regulations for hospital level of care (a Condition of Participation) and carry out a set of functions that ensure that the hospital stay is appropriate and necessary, and the level of care is correct. Will collaborate with nursing/medical staff and other providers to ensure the timely delivery of high quality care and is delegated to carry out regulatory requirements for the assessment of and communication to the patient by the hospital for all acute inpatient and observation status statutes.
Qualifications: Current Massachusetts R.N. License. Bachelor’s Degree required; BSN preferred. Certification in Case Management preferred. Minimum 7 years demonstrated experience as an RN in the acute hospital and/or home care setting; 2-5 years’ experience as a Nurse Case Manager preferred. Proven excellent interpersonal and communication skills. Ability to maintain confidentiality. Proven collaboration skills with other disciplines, ability to work independently, and excellent organizational skills/problem solving ability. Demonstrated ability to establish and maintain relationships with physicians and other healthcare professionals. Computer experience necessary-ability to use screening software, internet-based programs and documentation and data systems
