Position Summary:

Per Diem Clinical Pharmacist

This position’s responsibility is to help patients and other health care professionals, as well as the health care system to efficiently achieve optimal therapeutic outcomes. Key performance areas of responsibility are Clinical, Operations/Distribution, Financial, Safety and Regulatory, Research and Education.Provides comprehensive medication management services that integrate clinical services, order management, patient/staff education and research for employees and outpatients. Ambulatory pharmacists are responsible for improving the patient’s experience, safety, and achieving positive clinical and financial outcomes. This position is responsible to an immediate supervisor with co–supervision provided by the management team.Major Responsibilities:
  • Participates in drug therapy management and documents the care provided in patients’ medical records. Participates in interdisciplinary reviews of patients’ progress, as well as communicates pertinent patient information to other health care providers when patients change care settings to ensure continuity of care.
  • Participates in Medication Therapy Management (MTM) services, bills for services provided, identifies and addresses barriers to patient adherence to medication regimens as well as monitors the patient to assess the outcomes of medication therapy. Initiates or modifies medication therapy care plans on the basis of patient responses.
  • Recommends and order necessary laboratory tests, interpret test results, and communicate relevant issues to physicians and other team members.
  • Assists patients in gaining access to medications (e. g. , working with payers to resolve issues that may impede access to medication therapies) when applicable.
  • Interprets and responses to clinical decision support and drug utilization review from internal and external systems on patients drug therapy.

Position Qualifications:



Doctor of Pharmacy (Pharm. D) degree or Bachelor of Science in Pharmacy (BSPharm) degree. Licensed in the Commonwealth of Massachusetts.



3 years ambulatory experience required. Must have essential skills to implement motivational interviewing skills to assist the patient in taking an active part in their health care and promote healthy lifestyles. Duties involve normal pharmacy physical activity. Must be able to cross-cover in a variety of practice areas within the pharmacy as requested by supervisor.
