Description
| This position uses the nursing process to plan, implement, direct and evaluate patient care to achieve optimal patient outcomes; works with all other disciplines on assigned unit. |

- Analyzes data and assists with the development of action plans to improve quality care.
- Manages all aspects of discharge planning for assigned patients as follows: Meets directly with patient/family to assess needs and develop an individualized continuing care plan in collaboration with the physician.
- Provides patient with the appropriate teaching tool.
- Reviews documentation as needed throughout shift and ensures all documentation is complete on all assigned patients in a timely manner
- Accepts responsibility for effective two-way communication using clear and well-organized oral and written techniques. Demonstrates self-awareness of non-verbal messages, keeping verbal and non-verbal messages consistent. Incorporates recognition of cultural impact on understanding and effectively uses interpreters and language translation devices.
- Manages services to the patient throughout the continuum of care in order to decrease fragmentation of care.
- Updates patient information such as medication lists, allergies, etc
- Demonstrates excellent written and verbal communication abilities and creates mechanisms to facilitate effective organization-wide communication
- Assists with the management of efficiencies of unit activities to meet established productivity standards by prioritizing work assignments
- Serves as liaison between other nursing departments to expedite admissions/transfers; receives report and facilitates transition
