The Speech Therapist is responsible for implementation of standards of care for speech language pathology services.

Core Competencies

  • Evaluates patients to obtain and interpret data necessary for treatment planning and implementation
  • Analyzes evaluation results to identify the patients major problems, establishes goals with the patient, and develops and adjusts treatment program.
  • Identifies functional speech defects and establishes a plan of care to improve patient’s function.
  • Demonstrates knowledge of clinical norms for all age groups as applicable to job functions.
  • Maintains clinical records, clinical notes, and reports to the physician changes in the patient's condition.
  • Recommends communication devices/aids as indicated.
  • Attends and contributes to in-services, case conferences, and other meetings as required by Organization policy to ensure coordinated and comprehensive plans of care for the patients of the Organization.
  • Identifies patient and family/caregiver needs for home health services and refers as appropriate.

Department Specific Competencies

  • Establishes I will statements at annual performance evaluation and implements the statements.
  • Supervises and documents the home health aide plan of care every 2 weeks.


Additional Information
Possesses a degree that includes an internship from an approved Master's program in Speech–Language Pathology, or as required by applicable law and/or regulation. Meets the education and experience requirements for a Certificate of Clinical Competence in Speech–Language Pathology, granted by the American Speech–Language–Hearing Association (ASHA).
