Description

The Wound Care RN uses the nursing process to deliver nursing care to a given patient care population as it relates to the care setting. Nursing care is based on the interpretation and execution of the medical and nursing plan of treatment utilizing established procedures standards and protocols of individuals within the assigned work shift. The Wound Care RN provides nursing care to the population served in the unit.

Department Specific Competencies

  • Provides wound and ostomy care for those patients assigned and/or in immediate need of care, including the following activities: wound care per protocol and MD/NP order positions patients and changes position according to patients need scares for patients with special needs, eg traction, immobilization collects wound specimens as ordered by NP/MD performs patient/family education for wound and ostomy care. Performs treatments including the following: comprehensive wound assessment staging of pressure ulcers application of appropriate wound dressings, including Negative Pressure Wound Therapy application of binders and/or restraints application of appropriate ostomy pouching system and accessories.
  • Demonstrates Home Health specific competencies.
  • Administers medications. States the usual dose, action, and side effects of commonly used wound care medications uses pharmacist and NP for drug information not provided in unit references prepares and administers wound care medications follows medication administration schedules according to the patient care plan assesses patients for response/reaction to drugs/treatments labels multidose tubes/vials when opened assures outdated medications are discarded.
  • Performs OASIS/Comprehensive assessment and completes documentation in a timely manner.

  • Adheres to the guidelines regarding time points of specific questions, WOCN and outpatient infusion guidance.

  • Oversees coordination, communication and facilitation of the patient plan of care in a timely manner, while involving the physicians, staff and other healthcare team members

  • Submit weekly case management schedule to manager and coordinate care with interdisciplinary team, oversight of LVNs and CHHAs, and report any changes to manager immediately.

  • Supervises and documents the home health aide plan of care every 2 weeks.

  • Attends weekly case conference and staff meetings.

  • Adheres to telecommuting policy and obtains pre-approval prior to use the code.

  • Rotates on-call duties, responds within 20 minutes of receiving call, submits a call log next morning to the on call manager or team lead.

Additional Information
Candidates without national board certification in wound/ostomy care may be considered based on wound/ostomy training and experience, but certification would need to be obtained within 6 months of hire.

Compensation Range
$48.48 - $75.47 / Hour
