Plans, organizes, and provides medically prescribed and supervised exercise therapy, emotional support, community resources, and education for young, middle adult and geriatric patients.

Core Competencies

  • Provides all services of Cardiac Rehabilitation Program to adult patients.
  • Provides initial assessment and develops individualized care plans.
  • Assesses patients’ modifiable risk factors and offers support and strategies to aid patient in successful therapeutic lifestyle changes (TLC).
  • Utilizes contemporary theories of health behavior change in helping participants identify learning and lifestyle modification goals.
  • Provides education based on patient/familys learning needs, ability/readiness to learn and barriers to understanding.
  • Provides emotional support for patients and family while maintaining a positive motivating environment.
  • Applies evidence based principles of exercise science to develop individualized exercise prescriptions and adjusts prescriptions as necessary based on patient response and physician recommendations.
  • Supervises exercise session, monitoring participants for inappropriate responses to exercise, including blood pressure and telemetry.
  • Interprets cardiac rhythms and provide appropriate intervention.
  • Communicates effectively with rehab team members about individual patient findings or concerns.
  • Charts and documents initial and daily assessment data in participant records.
  • Reports any new, changing and/or significant participant conditions (dysrhythmias, abnormal blood pressure responses, abnormal blood glucose, change in participants’ status, etc) immediately to the patient’s physician or the physician supervising the program and charts.
  • Initiates appropriate interventions for adverse changes in patient’s status, including ACLS measures and/or emergency care according policy.
  • Consults with RN’s as needed.
  • Participates in the collection, analysis and reporting of identified individual patient outcomes and program outcomes.
  • Evaluates and initiates equipment repair and maintenance.

Additional Information
Bachelor's Degree in field of Exercise Science, Exercise Physiology, Kinesiology or the like.
