Description

The Occupational Therapist is responsible for implementation of standards of care for Occupational Therapy Services.

Core Competencies

  • Evaluates patients to obtain and interpret data necessary for treatment planning and implementation.

  • Analyzes evaluation results to identify the patients major problems, establishes goals with the patient, and develops and adjusts treatment programs.

  • Performs Occupational Therapy treatments based on evaluation results, aimed at increasing a patient's functional level.

  • Demonstrates knowledge of clinical norms for all age groups as applicable to job functions.

  • Maintains clinical records, clinical notes, and reports to the physician any changes in the patients condition.

  • Attends and contributes to in-services, case conferences, and other meetings as required by Organization policy to ensure coordinated and comprehensive plans of care for the patients of the Organization.


  • Identifies patient and family/caregiver needs for home health services and refers as appropriate.

  • Establishes I will statements at annual performance evaluation and implements the statements.

  • Supervises and documents the home health aide plan of care every 2 weeks.
