Description

The Manager Home Health, Nursing is responsible for ensuring that patient care is coordinated and managed appropriately. The Manager Home Health, Nursing is responsible for ensuring that care and services are delivered appropriately and for the supervision of clinical personnel in the delivery of patient care.

Core Competencies

  • Receives patient referrals from intake.

  • Reviews available patient information related to case, including disciplines required, to determine home care needs.

  • Uses clinical judgment to match needs of patient with skill of clinicians.

  • Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate.

  • Reviews patient’s clinical diagnosis, medications, procedures and clinical course.

  • Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.

  • Attends case conference meetings with organization personnel to facilitate coordination of care.

  • Creates and promotes a culture of safety, addressing high risk, problem prone areas.

  • Conducts quarterly record reviews and communicates findings and recommendations to Assistant Director and appropriate organization personnel.

  • Conducts periodic Tracers to assess continuous readiness for accreditation including hand hygiene compliance and conducts annual field competency joint visits.

  • Assists in the orientation of new organization personnel.

  • Assists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals.

  • Addresses personnel issues to promote a healthy work environment.

  • Utilizes the Just Culture Algorithm to coach, counsel and re-educate staff.

  • Conducts timely annual employee evaluations on clinical team members.

  • Develops monthly staffing schedule for team assigned and monitors violations related to time and attendance.

  • Assists Assistant Director in the planning, implementation and evaluation of inservice and continuing education programs.

  • Assists in the formulation, revision, implementation, and evaluation of policies and procedures, as well as strategic goals and objectives.

  • Complies with accepted professional standards and principles.

  • Participates in public relation and community activities that promote the Organizations role as an effective member of the health care delivery system.

  • Monitors team performance on HHCAHPS scores and assists Assistant Director in performance improvement of scores below the benchmarks.

  • Performs Manager Rounding follow up telephone calls on each new admission.

  • Promotes customer service orientation to all organization personnel.

    Additional Information
    Three years experience in the last five years in a home health agency, at least one of which was in a supervisory or administrative capacity.

    License / Certification Requirements

    Registered Nurse License
    BCLS or ACLS Certification
    Drivers license, auto insurance and reliable transportation
    National Cert as Home Health Mgr, OASIS or Other ANCC Cert in area of specialty within 1 yr of hire

    Compensation Range
    $116,334.40 - $193,211.20 / Annually
