Description
The Social Worker assists the patient and family to minimize stressors and problems that arise from social, economic, psychological or spiritual needs by utilizing appropriate community resources. |
Core Competencies |
- Interviews patients, family member and hospital personnel to identify and address needs.
- Assesses the psychosocial status of patients related to the patient's illness and environment and communicates findings to case manger and clinical manager.
- Provides counseling/support may be on issues related to: Death and dying, anticipatory grief, grief and loss, resolution of unfinished business, extent of care and resuscitation measures.
- Provides education related to: Use of resources understanding the patients medical condition/care, the normal grief process, organ donation, financial management (durable power of attorney, insurance), Durable Power of Attorney for Health Care/California Natural Death Act, Wills.
- Makes referrals to and coordination with community resources which may include; Clergy, family services, government programs, mortuary/funeral/burial information, homemaker/attendant services.
- Provides information and referral services for patients and families/caregivers and coordinates community resources regarding practical and environmental needs.
- Visits patients in their home to: Assess their psychosocial needs (initial and ongoing) including but not limited to: Social history (relating to present problem(s), cultural, educational, or religious issues, losses, and coping skills).
- Participates regularly in interdisciplinary conferences on behalf of patients.
- Contributes to the development of individual patient care plans.
- Provides support to patients and their families through individual or group conferences.
- Utilizes community agencies and resources.
- Prepares and keeps current case records for each patient including thorough and timely completion of documentation.
- Maintains clinical records on all patients which include initial evaluations and treatment plans; daily progress notes which reflect patient status and progress toward long and short term goals; interim re-evaluations showing treatment plan adjustments; case conference reports; and discharge summaries.
- Attends educational programs and professional meetings as they relate to the provision of social work services.
- Assists physicians and other team members in understanding patients significant social and emotional factors related to health problems.
- Keeps informed of changing Medi-Cal and Medicare regulations.
- Follows medical recommendations and provides service planned to restore patients to optimal social and health adjustment.
- Participates in hospital performance improvement activities (BPI projects, task forces, etc) when warranted.

Department Specific Competencies |
- Assumes the duties, roles and responsibilities of a social worker providing guidance, psycho-social counseling, and discharge planning assistance to patients and families.
- Helps patients/significant others identify community resources for follow-up care and support including facilitating contact with social worker.
- Makes complete initial assessment of the home care needs of the patient and family including physical, emotional and mental status. Gathers data on the social, economic and cultural factors which may influence health status of the ability to render hospice care in the home.

- Adheres to the National Association of Social Workers’ (NASW) Code of Ethics and JCAHO requirements.
- Provides new hire social workers with guidance, training, orientation and support, as appropriate.
- Composes list of resources for Case Managers and Social Workers as needed.
- Provides clinical supervision to Associate Clinical Social Workers (ACSW) per the Board of Behavioral Sciences requirements, as indicated.
- Seeks clinical consultation from Licensed Clinical Social Workers, as appropriate.
- Mentors, supports and provides consultation to clinical social workers, as needed.
- Attends weekly interdisciplinary conferences with the hospice team to discuss the emotional and physical impact on the hospice patient in order to provide a continuity of care for hospice families.
