Description

The TCU Admission Intake Coordinator is responsible for the evaluation of all new referrals to the Transitional Care Unit and coordination of the admission process. The person in this position will ensure requirements for admission to the TCU and continued through the patients stay are met.#LI-LP1

Core Competencies

  • Demonstrates knowledge of regulatory requirements as they relate to admission, continued stay and discharge from the TCU.
  • Ensures compliance with state, federal and regulatory requirements for admission to the TCU and continued through the patients stay.
  • Collaborates with referring case managers, both internal and external, as well as with referring physicians to promote a timely patient flow.
  • Maintains continued communication with referring case managers, both internal and external, as well as with referring physicians regarding needs of referred patients to meet requirements of admission.
  • Maintains knowledge of insurance payor contracting as well as authorization requirements by payor.
  • Collaborates with the Business Office regarding payment, pricing, and insurance billing.
  • Evaluates all patient referrals to the TCU according to the admission criteria per TCU Admission of Patients to the Skilled Nursing Care Unit policy.
  • Works closely with the Patient Access Department and the Business Office staff to ensure patients admitted to the TCU qualify for a TCU stay and funding source is verified.
  • Maintains communication with referring case managers or physicians regarding the outcome of the evaluation.

Department Specific Competencies

  • Works closely with TCU Leadership Team to manage bed flow and assignment, including movement of patients within the unit to meet patient and unit needs.
  • Ensures Medicare Certification and Recertification are completed in a timely manner and in compliance with regulatory requirements.
  • Participates in the Interdisciplinary Team Conference on a weekly basis to maintain communication regarding new admissions and continued stay requirements.

Two years of full-time experience as a licensed nurse, one year of which must be in the provision of direct patient care in a skilled/long term care facility.

$35.24 - $56.12 / Hour
