Duties:
- Provides professional nursing/mental health services to persons with a serious mental illness and intellectual disability living in the community
- Develops and implements, in conjunction with treatment team members, creative treatment plans responsive to client needs
- Identifies and coordinates provision of service and resources for clients, in collaboration with affiliate and community partners, advocating on client’s behalf to safeguard and ensure their entitlements and rights
- Conducts regular home visits to clients, requiring extensive driving in the catchment area
- Provides assessment and crisis intervention services
- Participates in the development and delivery of educational and informational programs for clients, families, service providers, employees and the public
- Provide necessary activities of daily living support within the framework of the FACT DD model based on client need
- Safe preparation and administration of medication as per the Royal policy and procedure
- Demonstrated ability to recognize and intervene around emergent health problems
- Work in a manner that is in compliance with staff and patient/client safety practices, policies and procedures of The Royal.

Qualifications:
- Graduate of recognized Diploma Program in Practical Nursing
- Current registration with the College of Nurses of Ontario.
- Knowledge of CNO Standards regarding RPN scope of practice.
- Understanding of MHA and relevant legislation required.
- Experience working with persons with Dual Diagnosis (Intellectual Disability and Psychiatric Illness) required.
- Community Mental Health experience preferred.
- Ability to apply critical thinking, problem solving, decision making, conflict resolution and prioritization skills to nursing duties required
- Knowledge of major mental illness from a bio-psychosocial perspective and experience in applying the psychosocial principles of rehabilitation and case management required
- Demonstrated ability to work effectively both independently and as a member of a professional inter-disciplinary team.
- Maintain work load measurement and data collection as required and input into the workload measurement system on a regular basis.
- WHMIS update in preceding 12 months.
- CPR Certification within the last 12 months.
- Completion of Prevention of Violence in the Workplace training (C.P.I.) is preferred.
- Basic computer skills required.
- Access to a reliable vehicle for day to day operations of the team
- Valid G License
- Excellent interpersonal and problem solving skills.
- Excellent oral and written communication skills.
- English level A- is mandatory in oral expression, oral comprehension, reading comprehension and written expression. Bilingual (French/English) is considered an asset.
Salary Range: | $35.36 to $37.05 per hour |
