What you’ll be doing:   

This role is the first step in the Revenue Cycle process which includes but not limited to either remote or onsite acquisition of the daily ED, UC or OBS log and chart after in-depth auditing and analysis, electronic submission via Kofax and Chart Receiver into SCP Health to initiate the coding and billing processes, and continuous resolution of strays, queries, and exceptions. Work to incorporate and maintain core values of agility, collaboration, respect and courage in all business decisions and interactions.
 
  • Daily reconciliation of Emergency Department, Urgent Care and/or Observation logs to patient charts to initiate coding and billing. 
  • Minimum daily chart production should be a minimum of 85-100 charts through manual pull or processing of stray and deficient charts. 
  • Cross trained to work at least 1 or 2 other sites. 
  • Utilize various methods of log and chart acquisition to include EMR, SFTP, DFS, scanning and feed sources. Properly format logs and charts into required Excel or other required formats, as necessary. 
  • Ensure log contains all required elements- facility name, patient name, account number, dos, MRN and CSN (where applicable) are configured into proper Excel format. 
  • Thorough analysis of entire chart following current departmental policy and procedures of all required documents and complete provider documentation; ensure chart has correct dos, account number and MRN which match the log; submit only required elements. 
  • Once chart batch is submitted via Chart Receiver, confirm receipt of charts in Kofax ensuring correct batch properties, legibility and quality documents, correct page orientation, deletion of unnecessary pages, split chart when necessary to adhere to page count max requirements. 
  • Accurate identification and assignment of deficient and non-bill charts ensuring these charts are not submitted until all requirements are met, proper use of only approved deficiency and non-bill reasons. 
 
Who/What we are looking for:   
  • Highschool diploma
  • 0-2 years’ experience in medical records
  • You are proficient in computer skills, Excel, and Word
  • You have a professional attitude and appearance to be able to communicate with hospital staff and providers.
