What you�ll be doing:

The Triage Nurse - RN is the clinical frontline of the Connected Care team.� This position is responsible for answering blended inbound and outbound telephone calls while working in conjunction with the administrative staff to manage patient care.

TheTriage Nurse - RNresponds to incoming telephone calls, voice messages and/or text messages regarding clinical issues.� TheTriage Nurse - RNhelps patients make health care decisions regarding the appropriate level of care, managing their general health needs, answering questions about tests, treatments, procedures, and healthy lifestyle changes. TheTriage Nurse - RNwill be responsible for utilizing clinical knowledge and the nursing process to quickly identify if one of our standardized clinical protocols is needed in order to safely resolve the needs of the patient.

  • Use critical thinking skills and respond quickly and professionally under stressful, sometimes emergent situations
  • Provide clinical assessment and nursing triage to a wide variety of patients across the health continuum and/or health education to culturally diverse populations
  • Assists in directing patients to the most appropriate level of care
  • Provide telephonic health information and nursing education
  • Assists and empowers patients to make better health care decisions
  • Helps patients to participate in other external programs or resources available to them to improve their health
  • Work in collaboration with Telehealth providers by referring appropriate patients who may benefit from a �virtual care visit�
  • Provide outbound follow up calls as necessary

Who/What we are looking for:

  • You are located in a state with a nurse compact license available and have a private, quiet place to work from home with reliable high speed internet service, required
  • You are able to work at least 10 hours per month; hours vary depending on shift selected, with the ability to work a full shift that ranges from 4 - 12 hours to meet the required 10 hours per month
  • You have a Nursing degree (diploma, ADN or BSN) required
  • You have an active, unrestricted RN licensure with at least five years of RN clinical nursing experience, required
  • You are able to be licensed under eNLC (Compact license) and reside in a compact state to acquire compact licensure, required
  • You have two years previous Emergency Department, EMT or Triage experience, required
    • Inpatient bedside assessment may be considered if in an acute patient setting
  • You are a team player with excellent verbal and written communication skills with all members of the healthcare team (nurses, physicians, care managers, social workers, etc.)
  • You have excellent organizational skills, can prioritize and complete tasks when there are multiple competing priorities
  • You are decisive and have attention to detail as well as proficiency with technology, including computers, software applications, phone systems, etc.
  • You have knowledge of Hospital Electronic Medical Records (EMR) systems
  • You are able to manage patient information according to HIPPA guidelines
  • You have ability to work under stressful conditions with irregular hours
