Our nurses are part of a 24/7/365 team of clinical and non-clinical Care Coordinators who have the ability to work within one of our Patient Engagement and Access Centers or remotely. The perfect nurse for us is one who is confident and capable of working independently and is an excellent communicator which means you are a good listener and can communicate information clearly and effectively. 
A day in the life of a nurse on SCP's Care Coordination team is filled with responding to incoming and outgoing telephone calls, voice messages regarding clinical issues. You will help patients make health care decisions regarding the appropriate level of care, educating on their general health needs, answering questions about tests, treatments, procedures, and healthy lifestyle change

  • Receives telephone and faxed referrals from Emergency Department.
  • Ability to use critical thinking skills and respond quickly and professionally under stressful, sometimes emergent situations.
  • Documents verbal orders given by the Emergency Department
  • Obtains demographic, clinical, and payer information from ER.
  • Evaluates patient and orders for appropriateness for home care.
  • Participates in the QA/QI plan and process.
  • Adheres to and participates in the Care Management process.
  • Ensures timely processing of intake information and documentation of follow-up.
  • Enters referral as a scheduled admit into EMR with appropriate information including request for services, pertinent history, patient notes, financial notes and insurance information.
  • Participates in staff conference regarding patient care.
  • Provide clinical assessment and nursing triage to a wide variety of patients across the health continuum and/or health education to culturally diverse population-including COVID-19.
  • Provide telephonic health information and nursing education related to target diagnoses for BCBS.
  • Assists in directing patients to the most appropriate level of care.
  • Encourages patients to participate in other external programs or resources available to them in order to improve their health.
  • Work in collaboration with Tele-health providers by referring appropriate patients who may benefit from a virtual care visit.
  • Ability to answer questions from patients following a tele-health visit
  • Interact with patients using a pre-determined call flow before and after surgical intervention
  • Patient outbound follow up calls as necessary.
Who/What we are looking for:

  • You are a Graduate of a US accredited nursing program with an active unrestricted RN license in good standing. Preferred field of study: Home Health, Case Management, Emergency Department
  • You must be a RN with a minimum of 5 years home health experience.
  • You must be very knowledgeable of the home health benefit and the requirements of homebound and medical necessity.
  • You have excellent customer service skills including professional phone voice, empathy, and active listening skills
  • You have excellent oral, written and interpersonal communication skills.
  • You must have excellent verbal and written communication skills with all members of the healthcare team.
  • You have experience in home health, emergency department, or case management preferred
  • You must have excellent organizational skills and ability to complete competing priorities.
  • You have decisiveness and attention to detail as well as proficiency with technology, including computers, software applications, phone systems, etc.
  • You must have thorough understanding of home health qualifying criteria and coverage guidelines.
  • You have proficient computer skills utilizing and navigating multiple applications at one time including Citrix, Outlook, MS office, VPN, and basic computer problem solving skills.
 
Certifications and Licenses:
  • Must possess current licensure as a Registered Nurse in the state of practice.
