The Hospice Clinical Branch Manager oversees day-to-day clinical branch operations and provides oversight of all patient care services and personnel of the location.  The Hospice Clinical Branch Manager possesses leadership traits and handles agency operations with professionalism, positive energy, proactiveness, and a solution-minded approach.  The Hospice Clinical Branch Manager coordinates care with the Interdisciplinary Team, patient/client, family, sales, and referring stakeholders to ensure continuity, appropriateness, and quality of services delivered.  The Hospice Clinical Branch Manager directs clinical staff on agency policies and procedures, compliance standards, and quality and timeliness of work.  The Hospice Clinical Branch Manager has a desire to grow the agency and drives a “white glove” Customer Service standard within the agency.  

Description
Reliant at Home is seeking a Full-Time Hospice Clinical Branch Manager based out of Reliant at Home’s Fort Worth office.  This is not a remote or work from home position. 

Must have two or more years of Hospice Branch Manager, Director of Nursing, or Clinical Manager experience, preferably in the Fort Worth market.  Must be proficient with EMR systems, preferably HCHB.

Essential Functions:
  • Directs the branch and facilitates the execution of superior clinical care, volunteer, bereavement, chaplaincy, and other programs that will bring value to Reliant at Home’s external selling propositions, organic recruiting, and community engagement.
  • Ensures the agency location and clinicians follow guidance on how to achieve optimal Star Ratings, patient satisfaction scores, and performance under value-based metrics.
  • Actively engages with field staff and makes visits as necessary for education, supervisory, coverage needs, and clinical support purposes. 
  • Leadership and communication style aligns with the culture priorities and values of Reliant at Home and The Bluebird Promise. 
  • Work collaboratively with the sales team and branch staff to proactively drive the importance of understanding the family and patient needs, timely responsiveness to referral source requests, team communication, and the execution of care coordination details that ensure impeccable customer service. 
  • Consistently work and communicate with Hospice Division Director and Reliant at Home’s talent acquisition team members to meet Hospice branch/location staffing needs and that they align with company-directed census and financial targets.  
  • Ensure location billing holds are processed in a timely manner and billing percentages exceed 97% by the first of each month.
  • Foster a collaborative team spirit and participate in the Admin on Call rotation with peer Clinical Branch Managers. 
  • Meet Reliant at Home Hospice patient service goals and maintain state and federal compliance expectations, executing on improving deficiencies identified through QAPI processes.
  • Ensure all HCHB and other required tasks are consistently completed.
  • Manage successful performance of agency KPIs such as census, field staff productivity, PPD costs, DME, and timely documentation.
