Summary: The PRN Relief Pharmacist provides intermittent, on-call coverage to ensure seamless pharmacy operations during staff absences, peak periods, or special projects. This position is not eligible for benefits and is scheduled based on organizational needs. The Relief Pharmacist is responsible for dispensing medications, providing drug information to patients and healthcare professionals, and ensuring the safe and effective use of medications. This role involves collaborating with the healthcare team to optimize patient care and overseeing the work of pharmacy technicians and clerks.
Areas of Responsibility
Medication Dispensing
- Accurately review, interpret, and dispense prescription orders, verifying dosage, route, and appropriateness.
- Perform comprehensive drug utilization reviews to detect potential drug interactions, allergies, and contraindications.
- Ensure prescriptions are filled in compliance with all applicable state and federal regulations and pharmacy policies.
- Provide guidance and oversight to pharmacy technicians during prescription preparation and
dispensing processes. - Ensure proper labeling, packaging, and accurate documentation of all dispensed medications.
Patient Consultation:
- Provide comprehensive medication counseling to patients, including proper administration, potential side effects, and expected outcomes.
- Respond effectively to patient inquiries and provide clear education on medication adherence and management.
- Educate patients on over-the-counter (OTC) medication choices and complementary therapies when appropriate.
- Assist patients with medication reconciliation and therapeutic decision-making.
- Identify and resolve barriers to medication adherence through targeted patient counseling.
Clinical Collaboration:
- Collaborate closely with healthcare providers to design and monitor individualized medication therapy plans.
- Participate actively in interdisciplinary team meetings to enhance patient care coordination as
needed. - Assist providers and clinical staff in managing and streamlining prior authorization processes.
- Provide drug information support to healthcare professionals to assist in clinical decision making.
- Actively participate in clinical pharmacy programs including medication therapy management (MTM) and immunization services.
Regulatory Compliance:
- Maintain precise documentation and compliance records for controlled substances per state and federal laws.
- Ensure all pharmacy operations adhere to applicable pharmacy practice standards and regulations, including 340B requirements.
- Participate in pharmacy audits, inspections, and quality improvement activities.
- Stay informed of legislative updates and proactively implement changes in pharmacy practices as necessary.
- Maintain comprehensive and current knowledge of pharmaceutical products, guidelines, and
regulatory requirements.
Staff Supervision:
- Support Pharmacy Manager to oversee daily activities and workflows of pharmacy technicians and clerks to ensure efficient and accurate dispensing.
- Provide training, mentorship, and performance feedback to support staff as needed.
- Support Pharmacy Manager in staff scheduling and workload management.
- Foster a collaborative and supportive team environment to promote staff engagement and professional development.
- Assist in identifying training needs and provide guidance for continuing education of pharmacy staff.
Education/Experience
- Doctor of Pharmacy (Pharm.D.) from an accredited institution required.
- Minimum of two (2)years of experience as a pharmacist in an outpatient, retail, or community pharmacy setting required.
- Experienced in managing Prior Authorization process is preferred.
- Proficiency in pharmacy management systems and electronic health records (preferably Epic).
- Must have working knowledge of 340B or able to complete 340B University within 90 days of hiring required.
- Bilingual (English/Spanish) professional with extensive experience in capacity building, trauma responsive service delivery, and program evaluation preferred.
Licenses and Certifications:
- Active and unrestricted Pharmacist license, in good standing, with the State of California required.
- Must have current immunization certificate (APhA program) or obtain it within 90 days of hiring required.
- Current Basic Life Support (BLS) certificate required.
- Candidate must be able to successfully meet PHC’s credentialing and privileging requirements.
Language Skills: Ability to communicate in English and Spanish with children and adults in person, on the telephone and in writing is preferred. Ability to effectively present information and respond to questions and requests from patients, coworkers, and others as necessary. Ability to effectively communicate direction, instruction, and guidance to pharmacists, pharmacy technician, pharmacy clerks and other healthcare providers.
