About your Role:
Our Developmental Home Program Coordinator assists in the coordination of operations of the Child Developmental Homes and Adult Developmental Homes program. In this role, you do the following:
Monitor homes licensed under Article 10 and 11 (Child and Adult Developmental Homes) and Title 21 (FFH) ensuring the licensing regulations are met. Licensing monitoring activities may include but are not limited to home visits one or two times monthly, amendments and annual renewals. The coordinator acts as Member advocate, assuring that Article 9 is maintained for all individuals receiving services from the Arizona Department of Economic Security/Division of Developmental Disabilities (DES/DDD) placed in an HRT Child or Adult Developmental Home and/or licensed foster homes with a DD Certification. The coordinator will write Behavior Treatment Plans as required by Article 9 for individuals residing in licensed homes and attend annual PRC meetings. This position establishes delivery system to State entities requiring monthly/quarterly documentation for individuals in placement in HRT homes including BTP data sheet, Progress Reports, Financial Records, Habilitation Outcomes data collection and Teaching Strategies. This Coordinator will also assist with DHP recruitment/ retention and partner with the DHP Supervisor on program specific projects.
What we are looking for:
  • Bachelor’s degree in a human service field (Social Work, Psychology, Human Service, Counseling, etc) and 5 years’ experience with DDD, child welfare, human services and/or families or 10 years’ experience with DDD
  • 21 years of age or older
· Ability to obtain and maintain a valid unrestricted Level One Fingerprint Clearance Card, DCS Clearance with no substantiated reports, Background checks as required, and First Aid/CPR Certification.
· Current Arizona State Driver’s License with a good driving record for the past 3 years, and current personal vehicle liability insurance and vehicle registration

Pay: $43,000.00 - $47,000.00 per year
