About the Program
The High Fidelity Wraparound (HFW) Program is an intensive, team-based care coordination model designed for children, youth, young adults, and families with complex behavioral health needs who are often involved with multiple systems such as mental health, child welfare, education, juvenile justice, and housing services. The goal is to help individuals remain successful in their homes, schools, and communities while reducing the need for higher levels of care.

High Fidelity Wraparound is an evidence-based, family-centered approach that brings together multidisciplinary team members, natural supports, family members, and service providers to create and implement individualized plans of care that build upon strengths, address barriers, and improve long-term outcomes.

Productivity Expectation:  This position has a baseline productivity expectation of 50%, generally equivalent to an average of approximately 3.5 hours of direct client service per workday.  The meeting of such requirements will be addressed in accordance with the Company policies and applicable law.

About the Role
The Housing Coordinator works closely with clients, families, treatment teams, landlords, housing providers, and community partners to identify and secure housing opportunities for individuals enrolled in the High Fidelity Wraparound Program. This position supports housing stability by helping clients navigate housing systems, address barriers, maintain successful placements, and access resources that promote long-term independence and community integration.

Responsibilities
· Collaborate with treatment teams to assess client housing needs and develop housing support strategies
· Research and maintain knowledge of local housing resources, affordable housing opportunities, and community-based supports
· Develop and maintain relationships with property managers, landlords, housing agencies, and community partners
· Assist clients with identifying, applying for, and securing housing opportunities
· Meet with clients to discuss housing options, financial responsibilities, housing stability, and lease-related concerns
· Support clients in maintaining successful housing placements and resolving housing-related challenges
· Coordinate with housing providers and stakeholders to address concerns and prevent housing disruptions
· Facilitate referrals and resource connections related to housing, benefits, transportation, and supportive services
· Organize and process housing invoices, payments, check requests, and supporting documentation
· Maintain accurate and timely progress notes, billing documentation, and service records in accordance with organizational and Medi-Cal requirements
· Meet established productivity and performance expectations
· Develop and maintain professional relationships with housing agencies, service providers, and community organizations
· Maintain compliance with HIPAA, ethical standards, professional boundaries, and organizational policies
· Transport clients as needed using a personal vehicle
· Perform additional duties as assigned

Required Qualifications
· Bachelor's degree in Human Services, Social Work, Psychology, Counseling, Sociology, Behavioral Sciences, or a related field
· Minimum of two years of experience working with individuals experiencing mental health challenges, housing instability, homelessness, or related populations preferred
· Strong organizational, documentation, and problem-solving skills
· Excellent interpersonal, written, and verbal communication skills
· Ability to work independently and manage multiple priorities
· Intermediate computer proficiency, including Microsoft Office applications and electronic documentation systems
· Valid California driver's license, reliable transportation, and active automobile insurance
· Proof of education required

Preferred Qualifications
· Experience working in behavioral health, housing services, homeless services, case management, or community-based programs
· Knowledge of housing resources, rental assistance programs, and community services
· Familiarity with behavioral health systems and rehabilitation principles
· Experience collaborating with landlords, housing providers, and community stakeholders
· Bilingual skills preferred
