About the Role
Clarvida is seeking a Care Coordinator I to support individuals and families by coordinating behavioral health, medical, and community-based services. In this role, you will serve as a central point of connection for clients, helping them access care, navigate systems, and achieve individualized goals that promote stability, wellness, and independence.

In this role, you will serve as a central point of connection for clients, helping them access care, navigate systems, and achieve individualized goals that promote stability, wellness, and independence.
This position is ideal for someone who is organized, compassionate, and comfortable working in the community while collaborating with multidisciplinary teams.
Responsibilities
• Coordinate care and services for assigned clients in accordance with individualized care plans
• Conduct assessments, service planning, and ongoing monitoring of client progress
• Link clients to behavioral health, medical, housing, employment, and social support services
• Advocate for clients and support engagement across systems of care
• Maintain regular contact with clients, providers, and internal team members
• Participate in interdisciplinary team meetings and case reviews
• Ensure timely, accurate, and compliant documentation
• Maintain confidentiality and adhere to all regulatory and agency standards

Requirements
• Bachelor’s degree in social work, psychology, human services, or a related field (or equivalent experience as allowed by state guidelines)
• Experience working with individuals with behavioral health, substance use, or social service needs preferred
• Strong organizational, communication, and problem-solving skills
• Ability to work independently in a community-based setting
• Valid Driver’s License and reliable transportation
• Ability to pass all required background checks
