Description
This position is dependent upon the successful acquisition and retention of grant funding; applicants should be aware that any failure to secure adequate grant funding may result in position elimination.

Under the general supervision of the Senior Director, the Referral Coordinator oversees the overall operation of the assigned program: Healthy Development Services (HDS). Responsibilities include but are not limited to: strategic planning, budget development and oversight, contract compliance, program development (planning, implementation and evaluation); subcontractor relations; and employee oversight (recruitment, hiring, training and performance monitoring). Key functions of this role include parent engagement and active outreach to providers and families, including how to access services; implementation and quality assurance of services provided under HDS as outlined in the HDS Implementation Guide; systems level communication with all providers working with an individual family (e.g. physicians, social worker, school, etc.) including timely and informative feedback on referrals to and from community partners; coordination, facilitation and documentation of a minimum of three multi-disciplinary team meetings per fiscal year with regional HDS providers and existing non-HDS partners (e.g. Regional Center, School Districts, CWS, PHN, etc.); implementation and quality assurance of data collection and evaluation for the region, including semi-annual HDS Data Outcomes Analysis Grid reporting; timely completion and submission of quarterly reports to First 5 San Diego via CMEDS; identification and documentation of unmet regional needs; and implementation of a process for the mental health screening of parents and/or primary caregivers and referrals for services, when appropriate.

Speak and read English at a level that is sufficient to satisfactorily perform the essential functions of the position. Knowledge of standard office equipment (i.e., calculator, fax, photocopier) and personal computer and computer software skills (i.e., MS Windows, Excel, Access, Word, PowerPoint, internet, e-mail). Windows computer skills including proficient use of keyboarding, use of mouse or keys for functions such as selecting items, use of drop down menus, scroll bars, opening folders, copying and similar operations required upon employment or within the 1st two weeks of employment to perform the essential functions of the job. Performs other duties as assigned. Follows Palomar Health rules, policies, procedures, applicable laws and standards. Carries out the mission, vision, and quality commitment of Palomar Health.

Job Requirements

Minimum Education: Bachelor's Degree in Social Work or Health Related Field
Preferred Education: Master's Degree in Social Work or Health Related Field
Minimum Experience: : 5 years in Healthcare related field with emphasis on outpatient services, maternal-child health, and/or early intervention; 2 years minimum experience in supervising programs, staff and contracts
Preferred Experience: 3 - 5 years' progressive supervisory experience
Prior financial management experience
Required Certification: American Heart Association recognized BLS - Heartsaver
