Description
With leadership oversight, the senior level team member FSS III develops, implements, and provides training, mentorship, home visiting support, reflective practice and administrative support meetings, data and evaluation review, program development, enhancement of digital infrastructure efficiencies, and training plans to enhance and develop program and home visiting staff competencies. Coordinates and implements home visiting staff client chart review to support Evidence-Based Model Accreditation and Annual Funder/Countywide Coordinator Site Monitoring Visits to support accreditation and annual compliance site visits. Identifies, coordinates, and provides staff support for a minimum of three (3) annual required or requested trainings/year, for example: Child Abuse and Neglect; Equity and Inclusion; and Professional Development. In partnership with leadership, evaluates, researches and develops program policy, protocol, and quality improvements relevant to the home visiting role. Enhances and expands program, community, and systems connections and coordination.
Provides intensive, ongoing home visitation to high-risk program participants with fidelity to the Healthy Families America (HFA) model, and curriculum, for example, Baby Talk (BT), and Mothers and Babies evidence-based models, in accordance with San Diego County and Countywide Coordinator AAP-CA3 program requirements. Recruits, enrolls, schedules, and supports client participation in all program activities, e.g., home visits, parent workshops, and special family events. Carry's a caseload of program families and utilizes reflective practice to build strong, long-term relationships with families to support healthy pregnancy and birth outcomes, age-appropriate child development, and family self-sufficiency, and to reduce child abuse and neglect. Implements family and child screenings and assessments at established intervals to identify family strengths, increase knowledge and skills to support child development, and connect families to community and state resources. Assists and supports program participants to set and meet short- and long-term individual and family goals. Attends program family activities with participants. Utilizes reflective practice and other strategies to build strong relationships with and connections between Family Support Specialists. In partnership with Leadership, and as assigned, conducts case review, reflective and administrative support meetings with Family Support Specialists, including PHNs as applicable. Provides measurable, objective home visiting documentation which clearly summarizes education provided during home visits, goals set in partnership with families, and resources provided to families. Responsible for timely and accurate input of client and family data into CMEDS, Palomar Health data systems, and other County or applicable data systems, on a daily and ongoing basis as appropriate. Engages in outreach activities under the guidance of the Supervisor/Director. Participates as a member of interdisciplinary and interagency teams. Consistent, transparent, comprehensive and proactive communication is required with Program Leadership, both verbally and in writing, to ensure timely role accountability; reporting of challenges and concerns, and program, client and staff updates; recommendations for program quality improvements, and enhancement of policies and procedures. Frequent travel required throughout North Inland and North Coastal regions of San Diego, and throughout San Diego as needed. Speak and read English and Spanish at a level that is sufficient to satisfactorily perform the essential functions of the position. Knowledge of standard office equipment (i.e., calculator, fax, photocopier) and personal computer and computer software skills (i.e., MS Windows, Word, PowerPoint, Excel, internet, e-mail). Windows computer skills including proficient use of keyboarding, use of mouse or keys for functions such as selecting items, use of drop down menus, scroll bars, opening folders, copying and similar operations required upon employment or within the 1st two weeks of employment to perform the essential functions of the job. Performs other duties as assigned. Follows Palomar Health rules, policies, procedures, applicable laws and standards. Carries out the mission, vision, and quality commitment of Palomar Health.
This position is dependent upon the successful acquisition and retention of grant funding; applicants should be aware that any failure to secure adequate grant funding may result in position elimination.

Job Requirements
Minimum Education: | Bachelor's Degree in Maternal/Child Health, Child Development, Social Work, Psychology, Public Health, Education or a human services/sciences field. Attended a minimum of 60 hours of specialized training (separate from required Family Support Specialist trainings), in applicable topics, such as, for example: Specialized Early Childhood Development topics, Advanced Reflective Practice, Strengthening Family Relations, Early Childhood Social-Emotional Behavior and Regulation Intervention Specialist Certification (EC-SEBRIS). |
