Role Description

This role will manage, organize, and communicate the details and statuses of payer contracts and provider credentialing across Oak Street Health teams. This individual will work cross-functionally and serve as the key liaison with both Field and Corporate teams on the completion of any provider transitions and other strategic initiatives as they arise. This is a fast-paced, high visibility role to ensure that we are enabling provider readiness and opening access to quality care at Oak Street Health for as many patients as possible. The Project Manager will coordinate across locations and functions to ensure timely and successful project completion.


  • Build and maintain relevant project plans, dashboards, status updates, and key metrics

  • Manage to completion the key steps required for all provider transitions around the company

  • Continuously improve how we handle provider transitions to ensure patient continuity and new center readiness

  • Work closely with functional and regional leaders to ensure priorities and local execution are aligned

  • Communicate and coordinate with stakeholders to ensure visibility and coordination among various workstreams

  • Other duties, as assigned

  • Quick learner comfortable with a high level of ambiguity

  • Bachelor’s degree required

  • US work authorization
