Title: Supervisor, Transitional Care Coordinator
Role Description:
Oak Street Health takes a team-based approach to providing outstanding patient care. Transitional Care Coordinators (TCCs) are an integral part of the team who help track patients across hospital and post-acute settings and help them get connected back to primary care. The Supervisor, Transitional Care Coordinator will provide day-to-day oversight and support to the TCC team. This includes but is not limited to: onboarding new TCCs, obtaining EMR access, assisting with workflow development, collaborating with Transitional Care RNs, Practice Managers, hospitals, community providers, and care teams, and ensuring the TCC team is exceeding expectations. We also expect you to be highly flexible, covering additional responsibilities, as needed. This will primarily be a remote work position.
Core Responsibilities:
Serve as Transitional Care Coordinator team lead
Coach associates and recommend methods to improve performance
Support team in identifying, designing, and implementing process improvements
Track and report on operational processes and productivity
Track resource issues and recommend solutions
Manage inbound notifications related to transitional care events from payer, vendor, facility, and other sources
Conduct outreach to inpatient and post-acute facilities to obtain information such as admission & discharge dates and discharge plans
Request and obtain medical records, routing documents to clinical staff for review, and uploading documents into electronic medical record platform
Conduct outreach to patients to schedule and confirm post-discharge appointments within electronic medical records platform
Coordinate with care team, facility, payer, and community-based organizations to ensure patientsâ transitional care needs are met
Other duties, as assigned

Experience in customer service setting (specific experience in healthcare preferred but not required)
Experience with care coordination and collaborating with an interdisciplinary team

Proficient PC skills
College degree preferred, high school diploma required
Fluency in Spanish, Polish, Russian, or other languages spoken by people in the communities we serve (where necessary)
US work authorization
