Role Description:
The Clinical Program Specialist supports program development, implementation, ongoing evaluation, continual process improvement and operational integration for the Behavioral Health Program. This position will specialize in supporting the Behavioral Health program, working collaboratively with program owners, clinical analytics, process improvement, project managers and subject matter experts. The position will act as a specialist on select program initiatives tied to the Behavioral Health roadmap responsible for supporting project plans, pushing project efforts, and maintaining appropriate documentation needed for key decisions and implementation. This position will partner with Clinical programs and clinical leaders to link operational performance to organizational goals facilitating ongoing evaluation and improvement activities.
Work on projects across OSH Behavioral Health program in support of value-based care initiatives and the Behavioral Health Road Map.
Assists in the development and maintenance of the Behavioral Health Program strategic roadmap, as directed.
Creates and maintains project documentation for Behavioral Health program initiatives.
Assists BH staff in the creation and ongoing maintenance of the process, procedure and training plan documentation for Clinical programs.
Supports the organization of training of clinical teams as well as other key areas within OSH on new BH programs and/or process changes.
Contributes to activities with shared responsibilities across departments.
Maintain documentation of specific reports for BHS patient panels for quality, coding, patient experience, and other population health metrics.
Works with BHS clinics to facilitate and coordinate resources and programs available through Population Health, and other external resources, to attain population health goals.
Supports the facilitation of population health and care coordination resources as needed to engage BHS’s and patients in the relevant population health initiatives.
Collaborates with the BHS management team to identify and develop the resources needed to meet population health goals.
Assists with the education and support of office managers and staff regarding BHS initiatives, resources, and processes.
What are we looking for?
We’re looking for motivated, experienced professional with:
College degree preferred, some college required.
A minimum of two years of combined experience in health care related work (or standard qualifications for a specialist).
Previous experience in working on projects, process improvements, training while working as part of a cross-functional or implementation team.
Ability to work effectively with Managers, Directors and above, as well as through the organization.
Excellent organizational skills, and solid business judgment.
Self-motivated, self-starter and ability to work independently as well as working effectively in a team environment
Excellent verbal and written communication skills, with strong attention to detail
Strong analytical thinking and problem-solving skills
