Role Description:
The Sr. Director of Pharmacy plays a key role at a time of significant growth and evolution for Oak Street Health. This position is responsible for ensuring the safe, effective, and efficient use of medications to enhance patient outcomes while integrating pharmacy practices into the overall value-based care model and aligning with organizational objectives. The Head of Pharmacy will serve as a strategic thought leader, defining the direction and strategy for pharmacy operations in alignment with organizational priorities and enhancing collaboration across multiple domains to improve patient outcomes and medication management.
To remain close to our patients and care teams and offer practical insights, the Head of Pharmacy should be willing and able to practice clinical pharmacy in our care model as needed and appropriate. The position will report to the National Medical Director of Population Health.
Core Responsibilities:
Overall Pharmacy Vision and Strategy Development: create and set a clear vision, purpose, and strategy for pharmacy services, encompassing medication management, chronic condition management, and adherence optimization.
Leadership and Team Management: provide direct oversight for pharmacy staff, including hiring, retaining, and developing a diverse and effective team of pharmacists.
Collaboration and Integration: partner closely with operational and clinical leaders across the organization to ensure integration of pharmacy services into the overall care model and achievement of organizational objectives.
Ensure Safety: Collaborate with clinical operations, medical leadership, technology team and population health team to develop and implement best practice related to medication safety
Measurement and Monitoring: Drive continuous improvement in pharmacy practices by evolving tools, measurement practices, and data-driven problem-solving across the organization.
Ownership of Organizational Outcomes: Together with operational dyads and other medical leadership, take ownership of key organizational priorities such as Part D Stars outcomes and outcomes PharmD driven programs for chronic condition management
Other Duties: Perform other duties as assigned by the National Medical Director of Population Health
What are we looking for?
Doctor of Pharmacy (Pharm.D.) from an accredited pharmacy school; active pharmacist license a state with an Oak Street Clinic or willingness to obtain.
Minimum of 5 years in a clinical pharmacy setting, with at least 3 years in a leadership role managing pharmacy services or teams.
Extensive experience in clinical pharmacy leadership, coaching staff, and promoting high-quality medication management.
Extensive experience in managed care, value-based healthcare, payer-provider collaboration, and CMS Part D Stars Program
Experience utilizing a metrics-driven approach to analyze cost, quality, and patient satisfaction data to inform pharmacy strategy and program redesign.
Self-aware, confident in leadership abilities, and eager to share expertise with a passionate, fast-growing team focused on healthcare innovation.
A strong commitment to serving the patient population and improving health outcomes through effective medication management.
Comfort with ambiguity and a strong desire to solve problems and innovate.
