Role description:
This position has direct supervision over Care Management teams and is responsible for providing leadership, planning, direction, and implementation of all aspects of the Care Coordination program. The Supervisor is responsible for ensuring team adherence to program standards, meeting metrics as set by management, and staff education both individually and on a group level.
Responsibilities:
Directly manage care coordination staff, including but not limited to, hiring, training, monitoring workload, conflict resolution, team meetings, preparing and conducting performance evaluations, and implementing performance improvement and disciplinary actions, when necessary.
Provide oversight to ensure compliance with program responsibilities and staff management.
Demonstrate ability to navigate multiple programs to identify priorities and opportunities for collaboration.
Ensure collaboration between Care coordination roles, supportive roles, and center care teams.
Possess knowledge of contractual requirements and, where applicable, State and/or Federal regulations for assigned programs and ensure compliance with requirements
Act as a subject matter expert in the review of program descriptions, policies, and program evaluations.
Review care coordination processes and key performance indicators to provide an individual assessment of compliance risk, internal control and the overall effectiveness and efficiency of the process.
Create training strategies, initiatives, and materials.
Create and maintain job aids for case management roles.
Consistently demonstrates compliance with HIPAA regulations, professional conduct, and ethical practice.
Under certain circumstances may need to support or carry a partial caseload.
Ability to travel to facilities, member homes, or other settings as needed.
Assistance with center activities, as needed.
Other duties, as assigned.
Qualifications:
Bachelor’s degree or Master’s degree preferred, with minimum of two years relevant health care experience
Knowledge of Medicaid/Medicare contracts and benefit systems is preferred
Demonstrated customer-focused interpersonal skills to interact in an effective manner with practitioners, the interdisciplinary health care team, community agencies, members, and families
Ability to work autonomously, follow through with delegated tasks and be directly accountable for deliverables
Outstanding verbal and written communication skills
Ability to work independently and maintain flexibility in a fast-paced environment
Excellent interpersonal and facilitation skills
Excellent organizational skills and ability to manage multiple priorities appropriately
Working knowledge of Microsoft Office Product Suite
Working knowledge of Google Suite
US work authorization
