Title: Supervisor, Community Health Worker

Location: Remote - Philadelphia or Detroit strongly preferred

Role Description

Are you passionate about fighting health disparities? Do you love building collaborative,

high-performing teams? We are looking for an energetic, organized leader with experience in community engagement and clinical social work or case management to manage a team of community health workers (CHWs). CHWs are natural helpers who share life circumstances (race, socioeconomic status, language, etc.) with the people they serve. Using an evidence-based model of care, CHWs provide structured advocacy, social support, and navigation to help their clients reach their health goals.

Core Responsibilities:

  • Recruit CHWs through engaging with local CBOs

  • Lead a team of CHWs who are supporting patients in the community and integration within clinics, hospitals, and collaboration with community-based organizations

  • Support and coach CHWs so they do their best work for clients

  • Analyze reports to ensure your CHWs are doing a good job

  • Integrate CHWs with care teams working in clinics and hospitals; collaborate with community-based organizations

  • Regularly partner with program leaders to strategize optimal ways to implement organizational initiatives

  • Champion organizational initiatives with CHWs to assist with understand how their work drives organizational goals and excellent patient care

  • Other duties, as assigned

What We’re Looking For:

  • Leadership experience with a non-profit or community-based organization

  • Experience with clinical social work or case management

  • Excellent understanding of the healthcare and social service systems including processes for enrolling in public benefits

  • Knowledge of the resources and community-based organizations in the community

  • Comfortable analyzing reports and using data to drive performance results

  • Excellent oral and written communication skills

  • College degree; MSW or MPH a plus.

  • Passion for fighting health inequality

  • Proficient with Microsoft Office and Google Suite

  • Detail-oriented with excellent problem-solving skills

  • Highly organized and independently driven with the ability to manage multiple priorities

  • Strong accountability and discipline against timelines and deliverables

  • US work authorization
