Role Description

The Associate of Population Health will immediately become an important contributor to the Oak Street Health team. S/he will execute a set of responsibilities to enable the success of our clinical model, which is core to Oak Street's success.

Responsibilities

Specific responsibilities include:

  • Key focus on health outcomes and controlling medical costs in our patient population
  • Owning the strategy, process, and implementation of key operational roll-outs (will involve on-the-ground support at our clinics)
  • Reporting and analytics in support of our Care Teams (physicians, nurses, care managers)
  • Analyses as needed on patient data from electronic medical record and insurance company partners
  • Market research on best practices related to value-based care
  • Establishing and maintaining relationships with both internal and external stakeholders
  • Other duties, as assigned

The role reports to the Director of Population Health and has significant opportunity for rapid advancement.

  • Unimpeachable integrity and maturity
  • 1-2 years experience at a consulting firm, similar analytics experience or healthcare organization
  • Strong MS Excel skills
  • Bachelor degree from top tier institution
  • US work authorization
