Purpose: Provides continuous digital observation and surveillance of assigned patients. First line of action is to verbally redirect the patient from engaging in at risk behaviors. Summons the on-site clinical staff if the patient requires assistance. Knowledgeable and supportive of patient and institutional confidentiality and related hospital policies.
 
Responsibilities
1. Demonstrates understanding and proficiency of the use of the Video Monitoring System (VMS) Solution software.
2. Maintains visual observation at all times. Verbally redirects patient over digital 2-way audio device that is in patient room. Immediately summons the on-site clinical staff if the patient requires assistance.
3. Demonstrates the knowledge and skill necessary to provide the appropriate care to patients based on their individualized treatment plan as delegated by a registered nurse.
4. Participates in hand-off of pertinent information/behavior about assigned patients when arriving on unit and upon completion of shift assignment.
5. Provides patient with explanations as necessary, but does not counsel or provide opinions. Demonstrates behaviors in line with Service Excellence.
6. Seeks help or advice as soon as possible when patient appears to pose a threat to themselves or others. Participates in a collaborative identification and reporting of patient safety issues. Assures patient environment safety. Seeks assistance with removal of any potential safety hazard from room and reports findings to clinical staff.
7. Demonstrates safe work practices and attitudes, follows safety rules, works to prevent unsafe conditions and behaviors, and participates in organizational and department safety programs.
8. Demonstrates understanding and proficiency of other digital health platforms for remote monitoring of patients as needed
9. Participates in cross training and coverage of other digital health platforms that fall within the scope of practice for a patient care monitor.
10. Ensure adherence to all HQ policies and procedures
11. Assist in the monitoring and evaluating of training program’s effectiveness, success and ROI periodically and report to Management
12. Collaborate with Management on resolving any specific problems and tailor programs as necessary

Required:  
  • High School Graduate/G.E.D
  • Minimum one (1) year experience in hospital or clinical environment (physician office, nursing home, ambulatory services, etc.) providing direct patient care.
  • Excellent verbal communication skills – most communication will be done remotely using a microphone and headset
  • Ability to communicate positively and effectively with patients and hospital staff, including assessing the situation and de-escalating if necessary.
  • Ability to multitask effectively, prioritizing multiple situations simultaneously and addressing each appropriately.
  • Basic computer skills, i.e. Microsoft.

  • Clinical Certification Strongly Preferred – CNA, Medical Assistant, PCT / PCA / HHA
