Title: Pharmacy Resident PGY 1 - Danbury Hospital Med Ed Pharmacy Residency - Full time/40 hours per week, Day/Evenings.
 
Summary: The postgraduate year one pharmacy resident position is a postgraduate training position. Allows the pharmacy resident to perform as a licensed practitioner but to train under the supervision of an experienced preceptor. Enhances general competencies in managing medication-use systems and support optimal medication therapy outcomes for patients with a broad range of disease states.

Responsibilities:

1. Performs all functions required to ensure proper medication management (as defined by The Joint Commission storage, preparation, dispensing, use and administration) according to accepted standards of medical, nursing, and pharmacy practice and applicable to hospital policy and/or law.

2. Performs all duties and activities necessary to fulfill competency areas (as defined by ASHP Accreditation Standards for Postgraduate Year One Pharmacy Residency Programs) assigned by Pharmacy Director, Pharmacy Managers, Pharmacy Residency Program Director, or Preceptors.

3. Provide safe and appropriate pharmaceutical care. Provides prescription services to inpatients/outpatients.  Checks unit-dose medication for appropriate drug, dose, quantity, and packaging integrity. Compounds IV medications including chemotherapy.  Facilitates availability of first doses and specialty items. Maintains adequate medication supplies. Verify medication orders for therapeutic appropriateness. Update medications on administrative records. Provide pharmacokinetic monitoring of medications. Monitor parental nutrition. Respond to emergency codes.

4. Promotes and supports safe medication management. Documents interventions related to drug therapy.  Dispense/provide information on investigational drugs. Maintain and enforce the documentation and security of controlled substance supplies. Monitor and report adverse drug reactions. Document all medication incident reports.

5. Develops and implements clinical initiatives along with policies and procedures. Share responsibility for formulary review and maintenance. Perform drug use evaluations. Evaluate activities of medication administration.

6.Provides medication counseling to patients. Provides drug information to physicians, nurses, patients and other health care professionals. Participates and contributes in patient care rounds. Attends departmental staff meetings and educational seminars. Contributes to pharmacist continuing education seminars. Provides in-service education to hospital health care professionals. Serve as a preceptor to pharmacy students.

7. Serves as a staff pharmacist as required by ASHP Accreditation Standards for Postgraduate Year One Pharmacy Residency Programs and WCHN Postgraduate Year One Pharmacy Residency Program.

8. Complete all requirements for completion of the Norwalk Pharmacy residency program. 

9. Fulfills all compliance responsibilities.

10. Perform other duties assigned.

Requirements:

  • Graduate of ACPE accredited Doctor of Pharmacy Degree Program required.
  • Must hold Connecticut pharmacist licensure or be eligible for licensure in Connecticut and must obtain a Connecticut pharmacist license within 90 days from start of residency program.
  • Failure to attain CT pharmacy licensure will result in dismissal from the Norwalk Pharmacy residency
    program.
  • The applicant must be a highly motivated pharmacist wanting advanced education and training that leads to an enhanced level of professional pharmacy practice.
  • The applicant must also apply through the PhORCAS system for application to the position and adhere to the rules of the resident matching program (RMP) process.
  • Minimum Experience: zero to six months

Desired:

  • Applicants with hospital pharmacy experience, knowledge of current standards  of pharmacy practice, knowledge of relevant Federal and State laws, considerable interpersonal skills, oral and written  communication skills, ability to maintain records, demonstrable teaching ability, a documented history of research and/or  publication experience, a history of presentations to a multidisciplinary and/or professional meeting audience, leadership  in professional organizations, and those with awards/honors within pharmacy and community service.
  • Preferred Skills in Cerner, Omnicell, Simply797, and Microsoft Office (Word, Excel, PowerPoint, Access). Become or is a member of a local, regional, and national professional pharmacy organization. Become or is certified in BLS and ACLS. Become or is certified to administered immunizations in state of Connecticut.
