LMSW/LCSW- Crisis Intervention Inpatient- Danbury, CT
Nuvance Health is seeking multiple per diem, part time, and full time LMSW/LCSW for the department of Crisis Intervention at Danbury Hospital.
Summary: Performs assessments and formulates discharge/transition plans in collaboration with patient, family and multidisciplinary healthcare team. Provides ongoing assessment, monitoring and evaluation to meet the individual needs of patients. Assists in resource management through facilitation of care and participation in program planning. Provides education to patients/families regarding coverage, care options and community resources.
Responsibilities:
  • Social Work Clinical Care Coordinators provide clinical social work assessment and interventions which include counseling, bereavement and crisis intervention as well as assessment and intervention in cases involving complex family/patient dynamics.
  • Assesses patient's clinical readiness for transition in collaboration with the healthcare team and identifies available hospital and community resources based on the patient's clinical needs. Initiate paperwork for timely transfer of information to the community providers. Maintains easy flow of communication regarding patient's development and progress to all involved in care of patient.
  • Maintains effective relationships and a positive outlook when interfacing with other departments, multidisciplinary healthcare team as well as patient's/families and community providers.
  • Acts as an intermediary between patients, physicians and community providing advocacy, accurate healthcare information, potential referral services and proper treatment plans. Initiates and coordinates patient care conferences as required.
  • Maintains confidentiality on relevant issues and information. Takes accountability for personal performance and goal achievement. Takes initiative to set high standards of honesty, integrity and performance for self and others.
  • Collaborates effectively with team members, develops good relationships both inside and outside of the team.   • In accordance with all regulatory requirements, documents accurately, objectively and timely in the patient's plan of care and discharge disposition.
  • Fulfills all compliance responsibilities related to position.
  • Performs other duties as assigned.
Other information:
  • Licensed Clinical Social Worker in State of Connecticut. Will accept other qualified candidates who have a Master’s Degree in Social Work or Clinical Counseling, with expectation to complete Licensure within one year of hire date to maintain employment.
  • Required: Broad-based clinical background and hospital or crisis experience. (Must be available to work a flexible schedule of days/evenings, weekends and holidays).
  • Minimum Experience: 3-5 years
Education: Preferred: Licensed Clinical Social Worker in State of Connecticut. Will accept other qualified candidates who have a Master’s Degree in Social Work or Clinical Counseling, with expectation to complete Licensure within one year of hire date to maintain employment.
Salary Range: $27.91-$51.83
