Mobile Crisis Social Worker Opportunity

Summary:

Performs crisis intervention services to include verbal, telehealth, and in-person screenings to evaluate level of risk and deliver appropriate support services. Completes assessments in the community and in the ED, when necessary, under the guidance of LIPs to assist with formulation of treatment needs and/or discharge/transition plans in collaboration with patient, family and multidisciplinary healthcare team.  This will be a temporary position for one year. (August 2020 - August 2021).  

 

Responsibilities:

 

1. Social Workers respond to Mobile Crisis Hotline calls and Outreach Requests from the community to provide assessment and interventions which include referrals for treatment, skills coaching, counseling, and crisis de-escalation in cases involving complex family/social dynamics. Adheres to all guidelines and performance measurement standards as outlined within the DMHAS grant, and recorded through timely online submission of DDaP service data. In accordance with all regulatory requirements, maintains documentation accurately, objectively and timely. (50%)      
2. Coordinates multiple aspects of care to all appropriate patient and referred clients. Makes the transition to the next level of care with the services required to ensure safety and promote high-quality outcomes.(10%)
3. Assists in the development of an individual plan of care and ensures the plan is implemented in a timely manner in collaboration with colleagues. Discusses with the healthcare team identified delays while providing direction, assistance and support in developing strategies and interventions to move patients/referred clients through the continuum of care. (10%)
4. Explores available hospital and community resources based on clinical needs. Initiate paperwork as appropriate for timely transfer of information to the community providers. Maintains easy flow of communication regarding progress to all involved in the care plan. (10%)
5. Maintains effective and collaborative relationships and a positive outlook when interfacing with other departments, multidisciplinary healthcare team as well as patient's/families, community providers, and police/EMS. (10%)
6. Acts as an intermediary between patients/referred clients, providers, and the community as needed to provide advocacy, accurate healthcare information, potential referral services and proper treatment plans. (10%).

7. Fulfills all compliance responsibilities related to the position.

8. Performs other duties as assigned.
