Purpose:
Following a medically prescribed physical therapy program, evaluates, treats and educates pediatric, adolescent, adult or geriatric patients who have disabilities, traumatic injuries, pain, disease or any dysfunction, and helps them reach their maximum performance and assume a place in their community while learning to live within the limits of his/her capabilities. Demonstrates knowledge and skills necessary to provide appropriate care to the age of the patient served in the inpatient and/or outpatient setting.
Essential Responsibilities
1. Provides direct patient care services to ensure the effectiveness of physical therapy programs while adhering to safety policies and procedures.
2. Evaluates patients and establishes an effective treatment plan to achieve set goals.
3. Provides physician prescribed physical therapy treatment to all patients within the assigned caseload, to return the patient to optimum and productive levels within the patient’s capabilities.
4. Prioritizes caseloads and available treatment times to provide for maximum efficiency.
5. Improves or minimizes residual physical disabilities of the patient by utilizing correct modalities of treatment and suggests variations.
6. Recognizes contraindications to continuing with scheduled treatment and consuls with responsible Physician.
7. Involves family or caregivers in treatment plan and discharge planning.
8. Interprets the appropriate information needed to identify each patient’s requirements relative to age-specific needs.
9. Performs appropriate documentation to effectively communicate the patients’ functional status to Physicians, insurance companies, the patient their caregivers.
10. Consults with hospital personnel to provide for quality patient care and cost-efficient programming and provides patient/caregiver education for home exercise programs and to prepare for discharge.
Education and Experience Requirements:
Bachelor’s Degree from an accredited physical therapy school.
PREFER: Master’s Degree or DPT Degree.

License, Registration, or Certification Requirements:
Active New York State Physical Therapist License or eligible with a temporary permit.
Working Conditions:
Manual: significant manual skills/motor coord & finger dexterity
Occupational: Significant occupational risk
Physical Effort: Very Heavy effort. May exert up to 50 lbs. force
Physical Environment: Some exposure to dirt, odors, noise, human waste, etc.
