Summary;
Coordinates and performs utilization review and patient medical record audits according to State & Federal licensing regulations and Joint Commission standards for home health. Observes for trends in data and assists in implementation of action plans to improve clinical documentation.
Responsibilities:
1. Coordinates and performs utilization review and patient medical record audits according to State & Federal licensing regulations and Joint Commission standards for home health.
2. Observes for trends in data and assists in implementation of action plans to improve clinical documentation.
3. Performs evaluation and improvement activities for quality review of OASIS assessments and related documentation; OASIS error reports; outcome reports, SHP database alerts, and Home Health Compare.
4. Assists in the preparation and presentation of educational training sessions for home care staff to improve level of staff documentation performance.
5. Coordinates the transmission of medical records/patient documentation to the agency coding vendor and manages the coding responses to include feedback to the clinical staff.
6. Conducts ICD-10 coding of patient clinical records as needed.
7. Participates in the supervisory weekend and holiday coverage.
8. Fulfills all compliance responsibilities related to the position.
9. Performs other duties as assigned.
Required:
Minimum RN diploma with 1 year home health experience in a state-licensed, Medicare-certified home health agency.
Proficiency in use of computers to include documentation in EMRs, Word, Excel, & PowerPoint.
Excellent verbal and written communication skills; ability to demonstrate effective critical thinking skills and problem-solving independently.
Adult BLS certification obtained from American Heart Association or the American Red Cross
Minimum Experience: one year
Desired: BSN; knowledge of quality management and performance improvement.

Physical Effort: Very Heavy effort. May exert up to 50 lbs. force
Physical Environment: Some exposure to dirt, odors, noise, human waste, etc.
