This position is eligible for up to a $5000 sign on bonus.

SUMMARY
This position is responsible for providing outpatient behavioral health services to patients of providers in the Community Health Centers of Roscommon, Prudenville, and Grayling offices.  In addition to outpatient services this position may provide coverage and support to Grayling Hospital patients for behavioral health needs.  

ENTRY REQUIREMENTS
Masters degree and license in human services field required. 
MSW or MA in psychology counseling preferred with 3 years or 6000 hours related work experience.

Fully licensed in good standing in the State of Michigan as a Mental Health Professional.

Minimum 2 years' experience.

*With manager approval, Limited Licensed individuals may be considered. 

Preferred experience with outpatient or inpatient mental health counseling;

Preferred experience in medical social work, discharge planning, and crisis assessment for psychiatric level of care placement.

Demonstrated knowledge and proficiency in best practice treatment of Mental Health Disorders.

Provides outpatient counseling services utilizing best practices to complete assessment, diagnosis, treatment planning, intervention, transition planning and evaluation of outcomes.

Documents all services from referral through disposition in accordance with hospital, departmental and regulatory agency requirements.

Provides hospital social work assessments and supports case management function in disposition planning for complex medical cases as needed.

Provides ER crisis assessment and level of care placement as needed.

Demonstrates competency through the annual competency assessment, customer evaluation, clinical staff dialogue, performance appraisals and staff education.

Communicates with other members of the health care team through case reviews to ensure continuity of care and coordination of services.

Identifies quality improvement needs and participates in implementing improvements.

Maintains effective communications with patients, families, team members and managers as evidenced by mutual support, open communication, trust and support.

Promotes good public relations with department personnel, physicians, other referral sources, patients, families and the community.

Assumes responsibility for continuing education and professional development by attending required in-services and other means of continuing professional education (e.g., readings, classes, workshops).

Adheres to all hospital and department policies.

Keeps manager and physician informed of activities, decisions and changes in patients' treatment and/or progress.

Provides the Manager with input regarding competencies and annual discussions, and assists with implementation of hospital and departmental policies and procedures.

Performs other duties as assigned.
