Job Title: Outreach and Engagement Specialist (Bilingual Spanish)

Department/Program: Outpatient

Reports to: Clinical Program Manager

FLSA status: Non-exempt

Location: Gresham, Beaverton

Salary: Bilingual Spanish - $25.60/hr. - $27.26/hr.

Job Summary:

The Outreach and Engagement Specialist is responsible for providing direct support to clients and their families enrolled in Outpatient MH services, including but not limited to, outreach/engagement support, service coordination, referrals, linkage, advocacy, case management, and discharge planning. The Outreach and Engagement Specialist works in conjunction with the MH therapists to assess client and family needs and develop a plan to meet those needs. This position provides clients and their families guidance and assistance in navigating complex systems in order to access services and resources that will support the client in meeting their treatment goals. 

 

Position Responsibilities and Duties:

  • Assesses needs of client and family members, identifying risk factors and creating a case management plan that outlines goals/objectives to meet the client and family needs.
  • Provides service coordination, referral, linkage and advocacy for clients and their families based on assessed needs.
  • Collaborates with caregivers, schools, hospitals, CPS, MH providers, Wraparound providers, and other entities to ensure client’s needs are met. 
  • Provides Client/Family outreach and engagement support.
  • Provides support and guidance to clients and their families during times of transition or crisis.
  • Conducts CANS assessments for clients with CPS involvement.
  • Supports Discharge Planning for clients.
  • Develops and maintains resource list, group therapy options, extracurricular activities, parenting resources, etc.
  • Develops and maintains community and external agency partnerships.
  • Conducts screenings for ancillary services and/or higher levels of care.
  • Provides psycho-education to clients and families.
  • Completes timely clinical documentation for billable and non-billable services in accordance with state, federal and organizational requirements.
  • Maintains efficient and effective clinical and administrative operation practices.
  • Adheres to Outpatient policies, procedures, and protocols and those outlined in the Morrison Child and Family Services Employee Guidebook.
  • Performs other related duties as required and assigned.
  • Demonstrate a commitment to developing a thorough knowledge and application of the Sanctuary model and other organizational policies & practices.  
  • Participate in staff development, in-services, and training related to equity and inclusion in the workplace; model appropriate behaviors; develop, recommend, and implement improvements to business practices with awareness and understanding of the impact in a trauma-informed and culturally diverse organization. 

Core Competencies:

  • Understands mental health assessment, treatment, case management, and service terminology.
  • Ability to perform administrative and organizational functions such as managing spreadsheets and reports, entering, and tracking data, scanning/uploading files, navigating multiple databases, and maintaining records.
  • Ability to read, record and interpret information including client insurance eligibility and information in Electronic Health Record (EHR) systems.
  • Behaves professionally (demeanor, dress, language, commitments, etc.).
  • Demonstrates the ability to recognize professional limitations and seeks supervision when appropriate.
  • Acknowledges and respects the diversity of all individuals and demonstrate openness to providing culturally responsive care and treatment to clients and their families.
  • Demonstrates knowledge and skill in assessing suicide risk factors and facilitating screening and triage.
  • Demonstrates sufficient knowledge to support clients experiencing a mental health crisis including de-escalation techniques and facilitating access to appropriate crisis services.
  • Understands and complies with mandatory reporting requirements. Use developmentally appropriate and trauma informed approach to follow agency protocol for assessment and intervention.

 

Required Position Qualifications:

Any combination of experience and education that would likely provide the required knowledge and abilities to be registered as a QMHA is qualifying. A typical way to obtain the knowledge and abilities would be:

  • A Bachelor’s degree in Behavioral Health or related field OR a combination of at least three years of education and/or experience in behavioral health field.
  • Bilingual in English and Spanish  Must pass a language test for Spanish.
  • Care Coordination or Case Management experience at a behavioral health or related agency.
  • Knowledge of state and county systems and resources.

 

Other Requirements:

  • QMHA-Registration within 30 days of hire
  • Personal transportation allowing for travel between program sites. If driving a personal vehicle, a valid driver’s license for state of residency, proof of personal auto insurance liability coverage, and a driving record that meets corporate driving criteria is required.  If driving a personal vehicle for work purposes more than once a week (4 times per month on average) must have and maintain personal auto liability coverage at $100,000/$100,000 level.
  • Bloodborne Pathogens training is required on first day of employment.
  • Provides emergency first-aid, as needed, and maintains a current standard first-aid/CPR certification.
  • Must pass criminal history check
