Description
- Utilizes the Nursing Process to perform the following functions:
- Assessment: Completes and documents holistic assessment. Formulates nursing problems and diagnoses.
- Planning: Initiates an individualized plan of care based on actual and/or potential patient problems.
- Implementation: Prioritizes nursing interventions. Demonstrates knowledge of standards of care and practice.
- Evaluation: Reviews and revises plan of care based on individual patient response.
- Communication: Effectively communicates with health care team to coordinate care and coordinates care with external case managers.
- Professional Practice Accountability: Independently pursues opportunities to maintain and improve competence in practice area.
- Leadership: Role models positive professional leadership skills and contributes to overall optimal functioning of Nursing Services Division.
