LSSI is hiring a full-time Social Services Coordinator, Monday through Friday from 8:00am to 4:00pm. Responsible for coordinating home and community-based services to residents and families assigned to senior apartment building.

Essential Functions:
  • Provide needs assessments, crisis intervention, and case management to residents while maintaining confidentiality.
  • Serve as a resource for the Housing Manager and maintain contacts in the community.
  • Arrange educational programs for residents and staff.
  • Maintain files in accordance with HUD and LSSI requirements and accurately complete all reports in a timely manner.
  • Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest and fair manner.
 Position Qualifications:
  • Bachelor’s degree in human services or related field required.
  • Background check clearance required.
  • Experience working with older adults and entitlements preferred.
  • Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing.
  • Excellent organization, presentation and pc/computer skills, including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.
  • Valid driver’s license, in good standing for the state of residency required.
  • Access to reliable transportation required.
  • Valid IL statutory minimum liability insurance coverage, bodily injury and property damage required.
