LSSI is hiring full-time Care Manager, Monday through Friday from 9am to 5pm. The Care Manager position is responsible for coordinating care and services throughout the transition process for assigned Williams/Colbert Class Members. Responsible for navigating any changes at the system/individual level, communicating clearly with relevant parties and advocating on behalf of Class Members to resolve conflicts and gain access to services in order to ensure a safe and expedient transition into independent living in the community.

Essential Functions:
- Review Comprehensive Assessment with Class Member and/or Guardian and provide discharge recommendations for Class Member.
- Complete initial intakes, including DHS documentation such as Comprehensive Service Plans and Risk Assessment and Mitigation Strategies.
- Ensure compliance and a safe transition into independent living for the Class Member, including completion of the Transition Checklist.
- Provide post-transition care monitoring, including home visits at interval identified by DHS.
- Complete Critical Incident Reports and ensure compliance with Care Manager and QA deliverables identified by DHS.
Position Qualifications:
- Master’s degree required.
- 1-3 years of clinical experience preferred.
- LCSW or LCPC preferred.
- Excellent written and verbal communication skills.
- Experience in Crisis Intervention & De-escalation.
- Excellent communication, organization, presentation and pc/computer skills (including proficiency with Microsoft Office – Outlook, Word, Excel and PowerPoint) along with other related software.
- Transport self to/from assigned nursing facilities, transport clients to/from appointments as needed, comply with traffic laws/regulations, strong commitment to safety, good navigational skills.
- Position requires valid IL driver’s license and insurance, required.
