LSSI is hiring a full-time CCS Intake Coordinator, Monday through Friday. Responsible for responding to all referrals from the CCS Case Work team (in-person & telephonic) and completes initial screening for new clients as well as scheduling initial intake appointments with the appropriate programs.

Essential Functions:
- Respond to all referrals from the CCS Case Work Team.
- Refer clients that do not meet criteria for LSSI services to care coordination for evaluation and Image to additional resources.
- Verify benefits and match insurance coverage to the appropriate service or provider.
- Maintain knowledge of LSSI programs and alternative community resources.
- Manage influx of new clients and program size.
- Schedule intakes and referral assignments with appropriate counselor.
- Utilize Electronic Health Record and client tracking systems.
- Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest and fair manner.

- Associates degree in counseling, social work or related human service field required.
- Background check clearance required.
- Demonstrated proficiency using electronic health record and client tracking systems.
- Experience with intake assessment and scheduling client appointments preferred.
- Demonstrated ability to communicate in a clear, comprehensible manner, bot verbally and in writing.
- Excellent organization, presentation and pc/computer skills, including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.
- Valid driver’s license, in good standing for the state of residency required.
- Access to reliable transportation required.
- Valid IL statutory minimum liability insurance coverage, bodily injury and property damage required.
