LSSI is hiring a full-time SOAR Counselor Case Manager, Monday through Friday from 9am to 5pm. Engage Williams and Colbert Class Members identified as having no income and work with Class Members to secure SSI/SSDI income in the manner prescribed by SOAR and DHS.

Essential Functions: 
  • Actively engage all Class Members referred to SOAR within 7 days of referral.
  • Gather all required documentation in order to complete and submit SSA applications and appeals for Class Members.
  • Write SOAR Medical Summary Reports for individual applications.
  • Coordinate visits to medical doctors, psychiatrists, and other specialists to obtain evidence for the application.
  • Coordinate case management services with partners and help with providing case management services to individuals when needed.
  • Ensure compliance with all SOAR standards and deliverables for DHS and the Williams and Colbert Consent Decrees.
  • Report on required deliverables to as required.
  • Other duties as assigned.
Position Qualifications: 
  • High School diploma required; Bachelor's degree preferred. 
  • Excellent written and verbal communication skills. 
  • Must obtain SOAR certification within 40 days of hire date.  
  • Proficiency in EHR documentation and review. 
  • Excellent written and verbal communication skills. 
  • Ability to work in a team environment. 
  • Excellent communication, organization, presentation and pc/computer skills (including proficiency with Microsoft Office – Outlook, Word, Excel and PowerPoint) along with other related software. 
  • Driving is required. 
  • Access to reliable transportation is required. 
  • Valid Driver’s License in good standing for the state of residency and insurance. 
  • Valid Illinois statutory and liability insurance coverage is required.
