At LMH Health, we are committed to being a partner for lifelong health for the communities we serve. The Wound Ostomy Continence Coordinator, RN coordinates and facilitates specialized nursing care for patients with needs for these skills.  These duties are performed in accordance with established policies, procedures and protocols of the healthcare organization, while complying with laws and regulations in maintaining confidentiality of patient and facility information.

This RN primarily supports inpatient care but may also be called upon to support outpatient needs as necessary.

  • Develop therapeutic relationships with patients and families promoting decision making based on the assessment of patient medical histories, symptoms, examinations, diagnosis, conditions and treatments.
  • Communicate and collaborate with providers of wound services at LMH both verbally and through documentation focusing on patients’ identified problems and needs. Develops and monitors patient care plans for skin and wound needs.
  • Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
  • Competently applies collaborative teamwork and leadership as a member of an inter-professional team to coordinate patient centered care for quality patient and unit outcomes of care processes by: Utilizing information, resources and technology.
  • Providing Patient Centered Care Nursing processes including Patient/Family, Evidence Based Practices (EBP), safety and process improvement.
  • Exemplifies professionalism: applies lifelong learning; contributes to the science of nursing; participates in governance; shares knowledge to expand the scale and scope of best practices with others; builds the ethical, moral and regulatory base for best practices and processes of patient and unit care.
  • Performs other duties as assigned in line with basic nursing practice with specialization in Wound/Ostomy and Continence care. This may include siting for ostomy placement prior to surgery.
  • Provides education to patients, nurses and clinical associates of skin care and prevention of pressure ulcers.
  • Leads and coordinates incidence and prevalence studies for inpatient services.
  • Performs other duties as needed or assigned.


  • Bachelor’s or Master’s in Nursing
  • RN with current licensure in the state of Kansas
  • Current Basic life Support for Healthcare Providers
  • Currently certified in Wound/Ostomy/Continence or to attain within an agreed upon timeframe.
